	In the event that my son/daughter requires emergency first aid whilst on a SAFA football course, I give permission for him/her to recceive first aid as considered appropriate. I understand that refunds are not issued and I accept that SAFA and its representatives shall not be liable for any injuries sustained by my son/daughter nor be liable for any loss/damage to their belongings. I understand that SAFA may photograph my child for promotional purposes (no names used) and publication in the local press and have no objections to this. I give permission for any photographs to be published via SAFA's website and made available for online sale to parents:
